READY TO POST// A-stan Reconstruction IG: lots of problems with Garzez hospital
— Purple Avenger

Other people are already jumping on the $500/gal payments made for diesel fuel, but as bad as that is, it was only a $300k rip off; mere pocket change as overcharging goes.

OK, that ripoff was newsworthy in its own right and the $500/gal figure certainly makes for good headlines and great one-liner standup jokes.

However, the most outrageous part of the Gardez hospital debacle is going largely unmentioned. You have to read a bit further into the IG's report to find it.

...The new 100-bed Gardez hospital is being built to replace an existing 70-bed hospital in the town of Gardez, Paktiya province. In an April 2013 audit report on health services in Afghanistan, we noted that the Afghan government may not be able to sustain the new Gardez hospital once it is completed.

We also found that the new hospitalÂ’s annual operation and maintenance costs could exceed five times the annual operating costs for the hospital that it will be replacing. For example, the existing Gardez hospital has annual operating costs, including generator fuel costs, of about $611,000, and USAID estimated that the annual generator fuel costs alone for the new hospital could be as much as $3.2 million...

Go ahead and read the whole report. Its not that long and its an eye opener. It references an organization called IOM as being mostly responsible for "lax controls" on the overcharging. I'd never heard of IOM before, but it sounds like a group you'd want to avoid like the plague in the future.

Old hospital annual costs = $8,700/bed.
New hospital annual costs = $32,000/bed.

Beware of Americans bearing "gifts"

If it's the International Organization for Migration, then I am familiar with them, though not for some years.

Dealt with them back in the 90s as part of assistance programs to the former Soviet republics.  Colleagues with more refugee background had worked with them all over the world.

As NGOs in that field went, they were pretty solid.  But each project is different - cannot shed any light on this apparent fiasco.  Though I will add that back then there was a fairly ingrained bias at USAID against capital projects (well-founded) - partly, I think, based on Afghan projects in the 60s/70s, ironically enough.

Too busy to look into it, but IOM was not really the sort to do capital projects.  They were more about building capacity for rule of law, dealing with refugees and migrants, etc.  Hey - they ought to open up some projects in the US, where an immigration-related collapse in rule of law is devastating the country!  (quietly in some respects, more openly in others)

On the IOM "About Us" page, it says they've been operating since 1951!  If they've been operating like this for the past 60 years, how the hell are they still in business?  Someone would've said something by now or noticed how money seems to vanish when they're running things.

Our IOM grants were fairly small, as they deal with, as I said, capacity-building (training locals, essentially) in process issues related to dealing with refugees, displaced persons, etc.  Pretty small potatoes, and often fairly well-spent, to the extent any foreign aid can be.  Teach a man to fish, etc.

My displeasure over this apparent boondoggle pales, though, alongside my cold fury and despair over all the American (and coalition, and good Afghan) lives that will end wasted because - above all else - the US is not a serious country, but sends (well, used to) its exceptional military to do serious tasks.  And this pre-dates the still-unbelievable debacle that is the Obamination - the degradation that made it possible to have ever HEARD of him is the same thing that will make Afghanistan such a waste.

33 Maybe they're building it with the knowledge it'll never run with all the equipment in place? How much "shrinkage" does a place like that experience over the course of a year? I'd imagine a week after an MRI is in place, it's sold to a hospital in Dubai, China or Russia for half price. If the genny doesn't have to power said MRI, it'll take a few less tank-fulls per month.

It means if you are eligible for a refund but didn't pay the penalty, they withhold your refund.

If you owe a penalty and paid income tax at the end of the year (i.e. no refund) and don't pay your penalty, they can't do shit.

84 But I suspect there will be many, especially those with decent coverage that is now being dropped, who will not like being shoved into Medicaid. Posted by: Margarita DeVille at October 24, 2013 01:50 PM (dfYL9) _________ I hear this often but don't get why. Every doctor I have ever been to accepted Medicaid. My kids' pediatrician which is one of the best in town, accepts it. People have this idea that Medicaid means your doctor is an intern that just graduated from the University of The Dominican Republic. That's just not the case at all. Every hospital accepts it, pretty much every doctor does too.

Posted by: Diogenes' Lamp at October 24, 2013 09:58 AM (bb5+k)

93 I realize that Obama's interpretation of the law is, shall we say, fuzzy. But that is for stuff that is fuzzy to begin with. Like, does it actually say anyhwere in the bill that the dates CAN'T be changed? No. However, it does states explicitly that the only way the penalty/tax can be collected is via a refund. It also explicitly says the IRS may not put a lien on ANY asset in relation to the penalty/tax. It's about as clear as it gets. If you go on the assumption that this will be ignored, fine. I just don't see it happening on this particular issue that could not be more clearly stated without any ambiguity.

116 We'd need more info to really say whether that gas was overpriced or not. It sounds normal to me. I've spent a lot of time in Afghanistan. They have no oil, no refineries, all of it has to be imported, and that's a very dangerous and expensive endeavor. There is a lot of security and personnel that goes into moving every little drop. The cost depends on exactly where you are. Getting gas out to Kabul is a lot cheaper than some remote FOB. For example, Russian contractors out there charge $60,000/hr for helicopter time, moving personnel or supplies. And they have full flight schedules.
