Breakthrough: Vaccine clears simian version of HIV out of monkeys
— Purple Avenger

Yea, I know, too soon, yada, yada, yada... ...but this is the first time I've heard of anything actually eradicating the virus in a manner that might be field deployable.

I've taken the liberty of offering some rough English translations for the medical'ese in the piece.

... Here we show that regardless of the route of challenge, RhCMV/SIV vector-elicited immune responses control SIVmac239 after demonstrable lymphatic and haematogenous viral dissemination, and that replication-competent SIV persists in several sites for weeks to months.
[Translation: AIDS is crafty and finds all kind of nooks and cranies to hide in]
Over time, however, protected RM lost signs of SIV infection, showing a consistent lack of measurable plasma- or tissue-associated virus using ultrasensitive assays, and a loss of T-cell reactivity to SIV determinants not in the vaccine. Extensive ultrasensitive quantitative PCR and quantitative PCR with reverse transcription analyses of tissues from RhCMV/SIV vector-protected RM necropsied 69–172 weeks after challenge did not detect SIV RNA or DNA sequences above background levels
[Translation: over time, no trace of the virus could be found in any of those usual sneaky hiding places]
, and replication-competent SIV was not detected in these RM by extensive co-culture analysis of tissues or by adoptive transfer of 60 million haematolymphoid cells to naive RM. These data provide compelling evidence for progressive clearance of a pathogenic lentiviral infection,
[Translation: we shot uninfected monkeys full of tissue from the infected/treated monkeys and they didn't subsequently get infected.]
and suggest that some lentiviral reservoirs may be susceptible to the continuous effector memory T-cell-mediated immune surveillance elicited and maintained by cytomegalovirus vectors...
[Translation: This seems to work on persistent viruses like HIV and may also work on other persistent viruses good at hiding, like Herpes]
[UPDATE: moar here]

6 Okay, question... if the virus is already there, is this a vaccine, or a "cure?" Because a vaccine is prophylaxis- you get it first and then the virus has a harder time taking hold (and therefore making you sick).

17 Okay, question... if the virus is already there, is this a vaccine, or a "cure?"

It appears to be a "cure".  I don't know if its prophylactic value has been examined.  Some cures are pretty nasty in their own right (ex. Rifampicin for TB treatment is nasty stuff and will beat the shit out of you while its killing off the TB), so their use as prophylactic wouldn't be recommended anyway.

19 Posted by: Purp PhD/MD/Esq/Nobel Laureate at September 24, 2013 10:04 AM (9MLX+) So isn't the term "vaccine" incorrect?

Posted by: AllenG (Dedicated Tenther) MFM Delenda Est at September 24, 2013 06:04 AM (/PCJa)

20 Since I hail from San Francisco, I can tell you that the prevailing sexual mores here boil down to this:

"I want to be able to fuck anyone I want, including total strangers, wherever and whenever and with whatever frequency I choose, in any and all orifices, without protection, while drunk and/or high on drugs, and in places including dirty lavatories. And when predictable medically unfortunate consequences accrue to me as a result of these habits, I will scream at the top of my lungs that hundreds of billions of dollars in other people's money must be spent on public research to make me well again."

So, this breakthrough will be big, welcome news to the locals.

Can't wait to see what the next big boom in venereal pathogens is going to be. Whatever it is, it'll get started right here.

That was the "early" necroropsy sample.  They were demonstrating the virus had made its way into its hiding places.

Those who continued treatment for 69–172weeks turned up clean...even in the hiding places.  So it was actually a "cure", although it takes some time.

115 What did Putin see in Obama that let him know he had nothing to fear from him?
He doesn't hunt.
There is a difference between a man who hunts and one who doesn't.
It indicates one is part of the male power hierarchy and how you can/will advance. Hunting is a part of that and is a strong indicator of male abilities.
Yes, there are other ways to navigate up the male power hierarchy.

However, Putin saw fear, weakness. He saw someone who would be too scared to pull the trigger even if he was starving. Instead of strength, he saw someone more used to poisons, more comfortable playing with words and having others do the hard work.

And then he knew he had him--because they (his power base) don't admire Obama, they only want a share of his power; he maintains power by what he can dole out. Not out of loyalty or admiration or inspiration. And they are always hunger for a new strongman.

So, all he had to do was threaten his mask of power, make him look weak before his base, and he would cave. He laid down very careful insults, insults that would be laughed off as diplomatic wrangling by a more powerful man, but deadly bait to Obama. Because--even though he is Russian--Putin was sending a message: "There is one in the world more powerful than Obama." That could not stand. Worse, by calling his bluff, calling Obama a liar, he teased that he would go so far as to make Obama's base no longer fear him. For who fears a liar?

Putin knows that Obama's weakness is not just his character--for he does have a lot of flaws--no, his biggest weakness is his power base. Obama has to appear to be the strongest man in the room all the time, no matter what.

And that is why Cruz is so dangerous.

Obama is the strongman who holds on by fear. And that fear is reinforced by making sure no one challenges him; a big show and careful facade of how everyone else fears him too.

By making Obama fight, he has already weakened him. And any weakness is fatal . . . .

Pretty damned easy, if you are African.

The only place in Africa where the scourge seems to have let up is in Uganda, where the population is mostly Catholic and abstinence is preached. All the other places are supposed to be using condoms, but that apparently hasn't stopped transmission rates. Whole families are succumbing. It is truly the scourge of the continent.

Here, not so much. Transmission to straight folks is mitigated by condom use or just not having sex with men who have sex with other men.

No no no no no no no.

She should not go anywhere NEAR Vallejo. If there's anyplace worse and more dangerous than Croak-land, that is it.

It's one of the poster children for California civic dysfunction. To include having gone bankrupt due to lavish spending on its unionized public employees. They have had to cut way back on policing as part of the bankruptcy.

Couple that to a massive gang problem, and you have an avoid at all costs situation. Which is a real shame, because geographically and economically, the city could be a gem. There are even some very nice old Victorians, but not worth the neighbors.

Your friend needs to move inland from Oakland. Get her on the east side of the Caldecott Tunnel. Things are still reasonably safe in "Lamorinda", the Lafayette/Moraga/Orinda axis.

Posted by: KG[/i][/b] at September 24, 2013 06:37 AM (p7BzH)

144 Okay, question... if the virus is already there, is this a vaccine, or a "cure?" Because a vaccine is prophylaxis- you get it first and then the virus has a harder time taking hold (and therefore making you sick). Therapeutic vaccination is an actual strategy. When you vaccinate, you're exposing the immune system to something in order to elicit an immune response or change an existing immune response. Prophylactic vaccination is the most common form anyone encounters, so the prophylactic part tends to be an unspoken assumption on the part of most folks. All that said, I'm not familiar with SIV as an HIV model, so I have no idea how well this would transfer to humans. The problem that all attempts at HIV vaccination have encounter thus far is the ease of mutation in the virus, and that the immune response elicited by vaccination and infection is stymied by said mutation.

Kill all the an animals, cure all the diseases.

So far my simple thesis is still working as desired.

Although, I'm a bit cautious of animal work on AIDS though.  SIV behaves just a bit differently than HIV, which actually explains the "We've almost cured it!" rush of the 90s.  We didn't know this, so we had, in fact created several really great SIV candidate vaccines, that did absolutely bupkis in humans.

Now we know a lot more about what we didn't know back then, so hopefully will filter out bad results (and this seems to be a methodology for attacking the virus, more so than an actual candidate treatment).

OTOH, people will publish all sorts of crap to keep the grant funding going, and AIDS is up there in the funding levels.

180 ""A dear, dear friend of mine lives in Oakland - OK, technically Rock
Ridge, but it's still Oakland - and just had three muggings take place
within a block of her house over the weekend."

Also, Rockridge is one of those California tragedies, where it's a little oasis of comparative calm and competence that is quite literally surrounded on all sides (it's an enclave city) by the epic disaster that is Oakland.

This leads to situations where the reasonably good Rockridge police end up having to try to deal with the endless failures of the corrupt and inept Oakland police, as Oakland's gangsta-driven street thug crime wave inevitably percolates across the municipal boundary.

I'd consider Rockridge a reasonably nice place to live if it weren't for the Oakland problem. Which isn't going away. Not unless the Rockridge city council build God's own wall around the place.

1st, Find cable connecting mobo to switch and wiggle connections.  It may just be corroded connectors.

If that doesn't solve the problem, try taking the switch out of the equation and shorting the switch cable pins on the mobo with a tiny screwdriver.  Depending on the switch type and mobo design, you may need a permanent contact or momentary.  If the switch/button feels like it "clicks in", and stays in until pressed again, its probably a permanent, if it feels the same for on/off, its probably momentary contact.

If all that fails, it could be the power-up circuitry on the mobo is toast

Lots of diseases make it hard to work (and therefore put you at risk of losing your housing).

I never got why AIDS was special in the disease pantheon. (Or rather why it remains special).

Over time, however, protected RM lost signs of SIV infection, showing a consistent lack of measurable plasma- or tissue-associated virus using ultrasensitive assays, and a loss of T-cell reactivity to SIV determinants not in the vaccine. Extensive ultrasensitive quantitative PCR and quantitative PCR with reverse transcription analyses of tissues from RhCMV/SIV vector-protected RM necropsied 69–172 weeks after challenge did not detect SIV RNA or DNA sequences above background levels [Translation: over time, no trace of the virus could be found in any of those usual sneaky hiding places] That's the nasty, especially-sneaky thing about viruses though, you can't really see them to hunt them down, you just have to go Ape-Shit on the entire immuno system just to "keep them in check".

Just while we're on the topic of viral infections:

I have a patient who's in biomedical research. He's genetically re-engineered a rhino-virus to infect and treat prostate tissue. (As in prostate cancer.) According to his tests it's 100% effective without surgery. The problem is the FDA wants to fast track it and get it on the market....but wants him to PROVE that the virus cannot become airborn and create an epidemic that would effectively render men sterile. From a research standpoint how exactly can anyone prove a negative? You can infect one mouse and place him with 100 mice all of whom die of old age an never test positive. That still doesn't guarantee the virus can never mutate.

319 Posted by: tsrblke, actual real life PhD student ------ Sure, there are infectious diseases that we don't spend as much money on, but they aren't as deadly as this one. In general, a disease that has the potential to be spread from person to person is more dangerous (from a public health standpoint, which is all the government should be worried about) than one that isn't Plus, a lot of the gov. programs are examples of Government Program Immortality---they began when HIV was a brand-new threat we knew little about and couldn't effectively treat, and they've just lingered on the way government programs always do. I'm not saying that the gov. should spend more on AIDS research; I tend toward private/charitable funding of that, now that's it not a huge threat. I do agree that there is a lot of special snowflake politicking going on with funding. But, yeah, comparing Alzheimers with AIDS is apples and oranges.

320 Jenny,

I'm sorry for your loss. And you make a good point about infectious disease vs non-infectious disease.

I'm not in the "they deserve AIDS" camp. No one "deserves" a horrible disease because of bad choices.

My uncle died of emphysema. Horrible way to die. But at the end of the day, he did it to himself through a lifetime of smoking. He eventually quit, but it was too late.

Barring a public epidemic, I would always prioritize funding research for deadly diseases that aren't tied closely to lifestyle over ones that are.

The politics of AIDS just pisses me off to no end.

Guy sitting there with his wife, isn't going to say "I got it from Fred at the bar probably" or "probably from shooting up last weekend" He'll say, fucked if I know doc, must have been that old girlfriend of mine. Eh, not so much.

What this "peaceful nuclear energy" nonsense elides is that the Iranians DON'T NEED THE STUFF. They're afloat on an ocean of cheap hydrocarbon energy that will last them a long time to come. There is one, and only one, reason for Iran to have nuclear reactors, and that's to have a fuel cycle which cranks out weapons-grade fissionables.

CDC is remarkably dishonest with their numbers.  First, the ONLY category of stats for which they do not break down by gender is the supposed "straight" sex category.

The fact remains, the  VAST majority of hetero transmissions of HIV are male to female.  There are some reasonably well documented cases of female to male transmission, but  they remain exceedingly rare.

However, the bigger problem with CDC's numbers is that they base their stats on state/local health department data, which is spotty at best.  The most accurate numbers and rates for transmission are found when researchers do follow-up interviews with those who test positive, and  lo and behold, what they find is that  MOST of those cases of "heterosexual" transmissions turn into situations where people would not disclose  homosexual activity or IV drug use/needle sharing.

Pretty good, in that it prompted others to look for less controversial ways to generate stem cells from people's own skin and such, which reduces rejection to zero.

IOW, if the kerfluffle had never happened, those other avenues would have had less appeal and rejection issues would still be there with any developed treatments.

Because more likely the problem would be in the recipient. (i.e. immune over response, excessive infection of the vector, immune weakening leading to secondary infection of another pathogen.)
I wonder if he misunderstands the FDA's request for information.

Nope....guaranteed that's what the FDA is asking for. In this case each patient tests negative for prostate tissue and negative for the virus within 8 weeks of infection. BUT the FDA has never certified a viral form of treatment before so they don't know how to act. They're trying to CYA but they're not sure yet what all is required to get that done. All they know for sure is the negative flack they get when something like Thallidamide gets approved for public use.

I think they're similar but not the same.  You can be on the down-low but not have a beard.  And having a beard does mean you are on the down-low.  Though I also think the idea of a beard indicates that they are aware of their role as a beard.  But then you have married guys on the down-low and their wives are not aware of it.

499 Other stem cell research had been going on before...

The self-generated stem cell stuff is only in the past few years.  The Chinese technique of transforming skin is very recent...like a year ago.

Sure people have been looking at stem cells in a generic sense, but it wasn't until very recently they looked at using your own tissue to manufacture rejection-proof stuff
