— DrewM One of the debates during the ObamaCare fight and the recent campaign was the impact of Medicare cuts on recipients. ObamaCare supporters denied that the cuts would affect seniors since the cuts were on "providers" and not on "benefits".

The Medicare Advantage cut gets the most attention, but it only accounts for about a third of the Affordable Care ActÂ’s spending reduction. Another big chunk comes from the hospitals. The health law changed how Medicare calculates what they get reimbursed for various services, slightly lowering their rates over time. Hospitals agreed to these cuts because they knew, at the same time, they would likely see an influx of paying patients with the Affordable Care ActÂ’s insurance expansion....
ItÂ’s worth noting that thereÂ’s one area these cuts donÂ’t touch: Medicare benefits. The Affordable Care Act rolls back payment rates for hospitals and insurers. It does not, however, change the basket of benefits that patients have access to. And, as Ezra pointed out earlier today, the Ryan budget would keep these cuts in place.

See, cutting payments to providers doesn't affect benefits, apparently because providers will make it up in volume by seeing more loss leading patients or something
That sounds ridiculous to most people with any real world experience but that was the case ObamaCare supporters made. Now, if only there were a real world way to test who is right.
And thanks to the very same Sarah Kliff on the Post's website we do have a real world test case. Shockingly, the ObamaCare promises fall apart when faced with reality.
Cancer clinics across the country have begun turning away thousands of Medicare patients, blaming the sequester budget cuts.Oncologists say the reduced funding, which took effect for Medicare on April 1, makes it impossible to administer expensive chemotherapy drugs while staying afloat financially.
Patients at these clinics would need to seek treatment elsewhere, such as at hospitals that might not have the capacity to accommodate them.
“If we treated the patients receiving the most expensive drugs, we’d be out of business in six months to a year,” said Jeff Vacirca, chief executive of North Shore Hematology Oncology Associates in New York. “The drugs we’re going to lose money on we’re not going to administer right now.”

“If you get cut on the service side, you can either absorb it or make do with fewer nurses,” said Ted Okon, director of the Community Oncology Alliance, which advocates for hundreds of cancer clinics nationwide. “This is a drug that we’re purchasing. The costs don’t change and you can’t do without it. There isn’t really wiggle room.”

Wait what? Cutting payments for services leads to less...services? Economics...how does it work?
Of course Medicare recipients will have the same "benefits" as liberals always promise, they just won't have anywhere to redeem them.

I remember as a child my grandfather saying on occassion he wished he was dead. As a 9 year old or so, I'm like WTF Gramps? But now as an older person seeing this great country going down the shitter, etc I can understand his feelings. At least for us there won't be any medicare, etc so death may come earlier.

Problem is of course even if we tinker with the system you still won't get anything intelligent out of it. Take your average HDHP, it's going to have a deductible of somewhere between 5k and 10k (a few 2.5ks are classified as that, but for now let's stick with the 5k option.).
So you get cancer, every freaking year you're going to have to shell out 5k to satisfy your deductible. I think this scares a lot of people. (And some people start on a HDHP then jump ship if they get actually sick.)
Even if you plan really well, I think having 10-15k in a HSA is possible but rare, meaning you'd drain it in 3 years max.
So risk averse people stick to the soup to nuts insurance plans (and who can blame them.)
We need something more comprehensive with life time out of pock maximum values that trigger in case of truly catastrophic illness, but nothing even approaches that. (We also need a culture change that says "yeah, at 75, you don't need a 90k prostate cancer treatment that extends your life by 2-3 months.)
Or to put it in Catholic terms (which I know best) we need to restore the language of "proportionate" and "disproportionate" care. (this decision is situated on the individual.)

Good question. I'm guessing a little of both but mostly just the actual expense of R and D and manufacture is the biggest cost. I know that my cocktail wasn't made until I was sitting in the clinic chair waiting for actual administration. The clinic was desperate to not waste a single drop. Each treatment was several thousands of dollars just for the meds. It was quite eye-opening, and my particular mix had been settled science for many years. IOW it wasn't a novel a therapy and was quite basic and routine as far as chemo goes.

The flipside though is the American consumer is an idiot. Get rid of the FDA and see the market flooded with moronic shit that doesn't do anything (but patients want). See Also: "Anti-neoplastons" which Burzynski managed to sideskirt the FDA on and pockets thousands selling to desperate cancer patients.
Reform the FDA is a better option I think, especially with regards to generic drugs.
Also there's the weird issue of who gets sued when something goes wrong. There's a case moving through the courts right now where a generic drug manufacturer is being sued because the drug had side effects which weren't discovered until after the generic went on the market. The side effects are the result of the generic, but that's what everyone was taking at that point.
So who gets sued? The generic maker who did everything they were required to do (remember, generics aren't tested for new side effects, just that they mimic the uptake of the name brand) or the name brand company that failed to disclose test results properly? Or no one?

Tell me about it. It takes a decade or more for a drug to come to market. I was slated to begin Tysabri infusion once a month for MS treatment the day the FDA pulled the drug from the market. I was pissed. All that money on R and D and poof! Gone. If I know the potential consequences of the treatment and sign a waiver then let me have the drug. Tysabri now has a black box warning and you have to jump through thousands of hoops to get it. God, I hate these people.

Obamacare has never been about healthcare. Any sick people made well as the result of Obamacare is purely incidental happenstance. Obamacare was designed to serve as a transitional step, a temporary placeholder until the time was right for implementation of a single-payer system similar to Great Britain's NHS. That Obamacare was slapped together willy-nilly seemingly at the last moment before its passage should've been the first clue. In fact, from a 'progressive' perspective, it's better that Obamacare is rife with bureaucratic complexity, unworkable exchanges, and unintelligible directives. Such failings furnish an argument to take it all the way.

The one's I'm looking at switching too don't have that option, the deductible resets every year as far as I can tell.
And they get really complicated in other areas. If my wife and I switch to it, and decide to have a kid in the next year or so, we'll drain any reasonable contribution on that event alone. So what's our incentive to switch? HDHP costs $200/month puts $1200 per year in a HSA. Comprehensive plan costs $350/month. Even if we put the extra $150/month into the HSA we'll only have 3k at the end of the year, which pretty much gets drained on Labor and Delivery if my math is accurate. So then we have to start all over, and hopefully start being able to get ahead next year.
So the system is rigged against new models of insurance.

Why fight this Obamacare thing when it may work out better in the long run?
The logical progression of this type of policy will leave us with a healthcare system similar to Argentina.
There will be a totally free, birth to death system for the masses in one tier.
The second tier will be clinics that treat members of unions or trade associations, set up and run by those organizations for the benefit of their members.
The rich will have something like the Swiss Medical Clinics that serve those with money.
People who use the free government run system won't receive the very best in care, but that's because no government can afford to give every person the ultimate in healthcare. Those in the middle will get better care, but only to the limits their union or association set, then you're back to the government system.
As always, the rich will get whatever they want. As it should be.
Why not just embrace what is the natural order of things?

Like I said we need a culture change. It's going to have to be centered on the individual though. I've seen people willingly bankrupt their family to extend their life a few months. Shouldn't they have considered their family's future in that choice before they made it?
As long as the "payer" is some anonymous 3rd party entity (or goverment) though we can't even begin to have this discussion because the answer will always come back "you're entitled to it!"

An oncologist, I used to own a small chain of private-practice clinics. Because I anticipated this fiasco, I sold out to a big corporation just before ObummerCare went through.
For 2013, CMS (Medicare) initially announced a breathtaking 19% overall cut in reimbursement to freestanding radiation oncology centers. After some lobbying, that was reduced to "only" 9% for this year, but with further cuts to come. Certainly, the equipment/service vendors, expert staff, mortgage lenders, and insurance companies (employee health, malpractice, & business liability) did not volunteer to match those cuts....
There probably will be little change in overall payouts by Medicare, though, because those big corporations are far more efficient and ruthless in coding (charging) than individual practioners ever were. The much-ballyhooed use of electronic medical records (EMR) produces computer-generated drivel which is ripe with opportunities for additional charges, but frustrating for clinicians trying extract actual new/useful information.

I had 10K in medical expenses (my costs only) this tax year and last. The Catestrophic coverage requires you to pay the first 5K up front then the insurance pays all after that. That is still cheaper for me.

The wife works at a top drug company. Its unbelievable what it costs to bring a drug to market and to keep producing it. Its not only that but all the stupid humans outside of the company you have to deal with. I can't even count how many multi multi million dollar drug shipments were lost due to complete fuckups during shipping. One airport kept leaving shipments on the tarmac to get overheated in the sun, or container ships that lost a whole row of containers, or 3rd world shithole countries that close up for some fucking national holiday and don't even have a single frigging driver to move the shit before it expires, or dickheads in customs that take too long. They lose millions and millions.

I'm not saying it doesn't work in all cases.
But those are actual numbers I'm dealing with. (Admittedly those are single person numbers. Not family, but the point still remains, and it only gets worse as you move to the family plan because the deductible and premium go up a lot, and the contribution to HSA doesn't increase by the same proportion.)
So based on those actual numbers, how the hell does one make it work? I actually asked my wife to ask that of her boss who apparently is on the HDHP. His advice is far more useful to me than the 50 year old single woman with 0 health problems who keeps telling my wife "it's totally great!"
As long as the economics of it aren't in my favor should I make a stupid financial choice just to prove a point?

Not at all. I just am sympathetic to those patients who are facing death who show up one day at their doctors office for their chemotherapy treatment, a treatment that they have received in the past, and are told that they can no longer receive it. When someone is literally facing death, I find it reprehensible that during the middle of their treatment, it is denied.
This is not hitting "the poor" or "minorities." It is hitting middle class cancer patients who need treatment or they will die. These are people that have had their treatments covered in the past, and now, on the whim of the government are being denied life saving treatment.
How would you feel if suddenly, your mother or father, who did not have the money to pay for chemo out of pocket, were told "oh, we're sorry, we can't administer you drugs any more, best of luck with that whole cancer thing." Are you going to shrug your shoulders and say "oh well, the government shouldn't be involved in healthcare" and watch your parent die?

In defense of LNP's, I've been under the care of two very good ones since my bypass surgery nearly four years ago. They're both close to my age and spent years on the floors as nurses prior to getting their certs. Both are well credentialed with all kinds of fancy letters after their names.
They have a doctor that oversees them (whom I've also met). They're compassionate and caring, and try to do as much for me as they can given their restrictions to only one area, the heart (yeah, it's a clinic, but run by the same people who run the hospital where I had my surgery).
I feel blessed to have what little care I have. I just hope it doesn't go away under OCare.

There is something wrong with medical education though. I know lots of medical students, talk to them all the time.
It's more of a problem in specialties, but over time you tend to see people less as "people" and more as "training tools."
You only need to look at the hospital that was doing pelvic exam training on anesthetized patients to see how this happens. (Good news is, I'm fairly certain they don't do that anymore. Bad news is, I've heard similar stories about different procedures by various people, so the tactic may change, but the attitude doesn't.)

Why are chemo drugs so expensive? Do ya think when Medicare stops reimbursing at the higher rates the price might magically drop? Some drugs, for some cancers, are probably used almost exclusively by Medicare patients, given that age is one of the biggest correlating factors in some cancers.
We have to remember that the root problem here IS Medicare. Medicare drives up the price of everything. All insurance does, for that matter. If medical care was an open market, prices for everything would be much lower.

I wish I knew, but there is the chance that manufacturing just does cost that much such that prices simply cannot drop to a significant degree. Sure, maybe a few cancers could be treated because a significant enough population has a particular "common" cancer and manufacture can scale to meet demand, but many other cancers won't scale and the expense will not induce production or research.
Some things are just exorbitantly expensive and are destined to remain so even under a capitalistic system. And under a socialist/communist one? Expense cannot even enter the equation as the incentive to produce at all is ground to dust.

Sunk R&D costs have to be recouped. Not just the ones for a given drug, but for all the ones that never made it to FDA approval.
Plus, fuckers in other countries' national health services buy drugs on a cost-plus basis, essentially dictating what they will pay. It's like saying you'll buy a copy of a movie for the price of an empty DVD, because that's the marginal cost.
That leaves sick Americans to pay the full freight of drug development. Drugs typically have about five years left of patent life when they're approved by the FDA, then they go generic. That means that all the drug development costs have to be recouped in five years, in one country.

Example: Canada.
Canada has price controls, so the national health care system won't pay above a certain price that they came up with using an algorithm worthy of an insurance actuarial thesis paper.
Because of the price controls, the same drug is cheaper compared to the US, and that's why a lot of seniors go on organized bus trips across the border and load up on their medicines. Canadian pharmacies do this at their own peril. Also, this means that the US is subsidizing the price controlled drug for Canadian citizens to some degree.

A certain large technology company in my area just had to re-tool their cadillac health insurance plans, so now the deductibles are really high per year ($4000-$6000), there are co-pays for everything where there didn't used to be, a bunch of medications and tests have been severely limited under the new insurance plan, as well as payments for certain types of doctor visits.
Plus, there is now a rather byzantine system to pay for health services, you either have "medical credit card" (which my ex refuses to give me for our son, for reasons which make no sense) or if you don't have that, you pay for the health service up front full amount. Third option is some weird 3rd party co-pay thing you can set up so now yet another company besides the service provider and the insurance company has their finger in the money pot.
And yet, I keep getting told that this company's health insurance is still the best in the country. If that's the case, I feel really sorry for other people because they must be getting screwed royally.

Steven O'Donnell, head of the Rhode Island State Police, tells The Associated Press on Thursday that there is no evidence of an active shooter.
The school in South Kingstown wrote on its Twitter feed late Thursday morning that it was investigating a possible gunman on campus. It told people to seek shelter and stay where they were.
O'Donnell says the gunman was reported at Chafee Hall, which houses several departments including psychology, journalism and economics and some college administration offices.
Messages left with URI spokespeople were not immediately returned.

I used to work as a QA tester in a pharma manufacturing company years ago, and I noticed that we were making batches of a drug I hadn't seen on the market before. I asked about it and my boss said it was for export only to Mexico and other third world countries. The drug left the US market back in 1975. They still made it because there was still a market for it outside the US where it was still cheap enough to buy and profitable to make in the quantities demanded.

WASHINGTON — In a long-awaited interpretation of the new health care law, the Obama administration said Monday that employers must offer health insurance to employees and their children, but will not be subject to any penalties if family coverage is unaffordable to workers.

Also, this means that the US is subsidizing the price controlled drug for Canadian citizens to some degree.
That's the bottom line. The drug companies figure they'll make up the loss in Canada in exchange for the much higher prices they can get in the US and in exchange for actually staying in the Canadian market.
If the US implements the same thing, all of a sudden the drug prices in Canada will shoot up.

Pharma makes some money in Canada, Europe, and Japan, just not all that much. The American market is as big as all the others put together, which is why pharma companies fight for market share here.
If the US implements price controls, overseas prices may shoot up, but probably they won't pay them. Then pharma will get out of the pharma business. (I used to work in pharma, and the idea of shifting to another industry used to toyed with from time to time. Sound incredible? Look at GE, which is now essentially a financial services company. That was considered one model.)

So I remember reading a long time ago and I don't recall the book (though I think it was Robert Frezza's A Small Colonial War) one of the characters said words to the effect that you got a right to eat, but that don't mean there is any food. Whole lotta people about to learn that lesson.
My 'bumper sticker' line to people when the discussion comes up is, "Notice how you never have a problem finding a dentist?" (dental, vision, prescription etc are, generally speaking, not funded through medicare).
Alternatively, "Food is necessary for quality of life. Perhaps the government should run the grocery stores?"
I cringe in anticipation of the answer to the second one.

Shortly before the last election I was with a group of very nice liberals (and well-off) my parents age. One of them has a business background, someone who really ought to understand this. But it was like running into a brick wall. Couldn't see the contradiction between "not cutting benefits" and cutting Medicare reimbursements. It was the literal illustration of my old psyche professor's line "you can't argue with a delusion."

The Affordable Care Act rolls back payment rates for hospitals and insurers. It does not, however, change the basket of benefits that patients have access to.
The stupidity of this statement is astounding. So suppose ACA "rolled back" payment rates to zero. Patients would still have access to the same "basket of benefits," except no one would actually provide those benefits. It'd be like going to a restaurant with no food, but lots of menus.

The computerized health system overhaul is what really scares me. I have heard horror stories from my friend who works in a hospital. They had a perfectly functional system that they all knew inside and out, but were forced to change to a new system because of the computerized health care portion of ObamaCare. Because of the way certain things are on drop down menus, and the close proximity in numbers for certain tests, the doctors have already many times ordered the wrong test be performed.
I know some folks here work on the medical systems so maybe my perception is wrong, but it kind of sounds like from what she was telling me, the people who designed the interfaces weren't necessarily front line emergency room workers and so didn't take a lot of needs in that setting into account. The system seems to work best in a low stress setting and for people who spend a large part of their day doing data entry, not saving lives.

I think it's partly fear. Fear of responsibility. It's already impossible here in the states to have a meaningful conversation about getting employers out of the insurance game, which is really the most logical thing that would fix a whole bunch of problems.

Agreed. When payer is insulated from the payee, all perspective is lost. This is actually what happened when income tax withholding was instituted. If individuals were required to write a check each month, or show up at the Tax Office with cash, there would be a revolt in no time.

Recent changes in how Medicare pays, caused by the ACA, have forced the owner to sell off the clinics to a national chain. Most kidney patients are chronically anemic because their kidneys do not produce the hormone that stimulates red blood cell production. They receive periodic shots of a synthetic form of EPO (the doping drug) to replace that hormone. It is very expensive, but extremely necessary.
Under the ACA, clinics can't bill for that separately. They receive a bundled payment per patient to cover all costs. For many clinics, they will lose money to the point that they can no longer stay open. These doctors sold out when they could.

Aside from Wolf spider hunting, you could take them to the carwash, the ones where you spray the car down with that gun-hose.
When I met my wife, she had three boys. One day I took them all to the carwash to wash her minivan. I brought $10 worth of quarters and let them take turns pretending to be a fireman and hose down her van. It was the greatest thing ever to them! They still talk about it today.

Yes, it's legal to carry a long gun openly in most places in Indiana. There are a few restrictions: courthouses, government buildings, x number of feet from polling areas, and so on (along with the usual federal restrictions). You can, however, be nailed on a brandishing charge if authorities think you're deliberately frightening people, so intent matters. For example, if you get in an argument with someone and show off your handgun (or wave your rifle, I suppose) in order to intimidate, then that's brandishing.

I like spiders, too. I had a Grammastola Rosea tarantula for four years. Her name was Betty. She had this honey-blonde coloring and these pretty little onyx eyes. She didn't do any tricks, but I liked having her around. I only handled her when I cleaned out her habitat but she was always peaceable, never a problem.
