Contrary to Obama's Claims, Insuring the Uninsured Via Medicaid Cause More to Go to the Emergency, Not Fewer

You've heard this claim sixty thousand times: The uninsured don't go to doctors before a condition worsens, so they show up at the emergency room with a condition that could have been treated much more cheaply earlier.

Or: The poor go to the emergency room because they know they can't be turned away. And thus the absolutely highest-dollar part of the hospital is overutilized, thus driving up health care costs. Which you pay for, because, in the end, the hospital isn't eating that loss.

So, Obamacare was supposed to reduce costs by weaning the uninsured off their habituation of using the emergency room as a local health clinic.

Nope. Medicaid patients use the emergency room forty percent more than the completely uninsured.

Is there a single claim about this program that wasn't a "wrong promise"?

A new study of Medicaid beneficiaries in Oregon makes a strong version of this case. The study, published today in the journal Science, finds that adult Medicaid beneficiaries rely on emergency rooms about 40 percent more than similar uninsured adults.

"When you cover the uninsured, emergency room use goes up by a large magnitude," said Amy Finkelstein, a health economist at the Massachusetts Institute of Technology who served as a lead investigator on the study, in an MIT press statement accompanying the study.

There were no exceptions to the trend. "In no case were we able to find any subpopulations, or type of conditions, for which Medicaid caused a significant decrease in emergency department use," said Finkelstein.

As Peter Suderman notes there, a previous Oregon Medicaid study also demonstrated that people on Medicaid experience no better health outcomes than those without any insurance at all.

And Obama care won't decrease them one bit. In Fact their will be an increase. Some accidental because they didn't know they weren't covered, some on purpose cause they figure the gov't or an insurer will pay for it at some point.

Most because they can. Why bother with pesky insurance premiums, signing up and filling out forms when you can just present yourself at the ER with your symptoms and BY LAW the hospital must admit you if you fit certain very flexible criteria. they may or may not CURE what you have but they will stabilize you and diagnose you.

Seen by a nurse immediately and the doctor must see you within 15 minutes of that.( These are quality measures you must prove to govt. or get your reimbursement cut.) Why should they go to a Doctors office and wait for hours and or get up early to make an appointment when they can show up at the ER at noon,or say 3 pm for that runny nose they've had for a week. That's why the posted signs stating EMERGENCIES will be seen before anyone. I have literally been in code situations where patients came to the door to complain about having to wait.Nevermind the poor guy on the table dying and yes they were all Medicaid patients they could care less that we are paying those bills.

Try to make something idiot proof and nature builds a better idiot. I saw an Emergency Room documentary thinger where a homeless crazy BIRM woman came in with some issue. She had lesions all over her, and was housing worms in the lesions. When the medicos wanted to remove them she freaked out. She wanted to keep them. So they came up with a bunch of plastic tubs and put the worms into the tubs for her, complete with the names of the worms on them. Then they scrubbed out the lesions and put bandages on them. I'm 99% sure she packed the lesions with dirt and worms as soon as she left the ER.

Not a lot of doctors accept Medicaid and I'm guessing that the top practice in specialty fields do not. (Just a guess.) So those on Medicaid end up with less-than-the-best in care. The uninsured, however.... watch out for their own health as much as possible, dreading the cost of a hospital or doctor's bill. When they must go to the doctor, they are spending their own money and are more careful about where they spend it.

I have medicaid and am regulated to a specific clinic BUT if I get sick suddenly and call to see if they can see me, they send me to the emergency room.  Seriously.  Then, after sitting there for 10-12 hours (illegal aliens!), I can get treatment.

 

I can't wait until all those who have been pushed on to Medicaid after having regular insurance experience that.  The last time I got sick, I went ahead and paid $70 bucks (borrowed) to see a doctor, which took 45 minutes max.

I worked in an ER while I was getting my EMT. Seen by a nurse immediately? Is immediately 30 minutes? Seen by a doctor in 15 minutes? You meant 150 minutes right? Few years ago I got a fish hook in my hand in such a way I couldn't get it out myself, went to er, five hours later finally got it cut out. Last year I gashed my arm open, fixed it myself in about thirty minutes then went back to working on my house.

Old and white, just a few years retired. Been healthy until a few months ago. Started having problems. Nausea, headaches, balance problems, anxiety problems. Finally she had a fall on Christmas. Rushed to the hospital. Inoperable brain tumor. 12 weeks to live. Nice to see you. Here's the door. Here's your hospice lady. Thanks for coming. Goodbye. No expensive treatments. Just hospice and morphine. Very efficient. Money saving for the State. Not gonna be a lot of expensive treatments for her. Medicare spent a couple grand on the MRI, I guess. From here on out it's pretty cheap stuff. Just hospice and morphine.

And nurses who know how to treat clinic pts. as clinic pts. Get em in and get em out so you have beds for the seriously ill.you know as well as I do that the backup cones when the doc orders the Ct-scan for the migraine to prevent lawsuits, or multiple x-rays to also prevent future lawsuits.

Anyway, took almost an hour, and I never saw a doc. I got 3 stitches from a PA & a $2600 bill. Spent over 3 hours there, and the place was empty.

163 Had the ex get sick once, after weeding the flower bed. Then progressively sicker really quick. Me, being a play doctor immediately thought it was due to her touching a mushroom in the flower bed that looked like a dog dick. So later we're in the emergency room, showing the doctor a picture of the dog dick mushroom, and prediagnosing her condition when he looked at me pitifully and said, its her gallbladder. SO I suppose they do come in handy. I was WAY off.

164 I've had good and bad experiences in the ER.  Unfortunately, I've spent quite a bit of time in them over the years.  The worst was when a guy with a grossly broken leg was sitting in a wheelchair, and I was sitting two chairs over from him writhing in agony from a kidney stone while the triage nurse tried to explain to a couple how to give tylenol to their baby who was cutting teeth.  I kid you not, they brought their infant to the ER because he was teething.  Meanwhile, the guy bleeding with a broken leg sat there and patiently waited his turn.  I left and went to another ER.  The worst part for me was I had gone to a clinic earlier in the day because I knew it was a kidney stone (I have a history), told the doc at the clinic it was a kidney stone, and he gave me a shot for pain relief and some antibiotics for the UTI he thought I had, but no prescription so I could make it through the rest of the day before the tiny grapenut of death passed.

170 Joanne no offense intended the vast majority of Medicaid patients never graced our ER but boy did we have the frequent fliers, we also had a lot of pts. that would give the name of our ER director as their primary care physician and tell us they refused to go to a regular doctor because he was the best. mind you we also had a clinic next door with many great doctors from pediatricians to gerontologists.

175 Yeah, maybe I am whining too much. I did my ER training five years ago in a small NC hospital. Mostly they were pretty damn good. I would have thrown half these people out on the street, they were so obviously faking shit. No one did, which impressed me. A lot of people come to the ER faking pain so they can score some free narcotics. Because of EMTALA you can't just turn a known frequent flier away at the door. I've also heard of homeless folks going to the ER and faking illness so they can get admitted, have a room for the night and a free meal. Again, you can't just refuse them, you at least have to give them a screening examination which takes time and resources away from people who are actually sick.

I give soon to be former employees, most have new jobs lined up, the letter with the COBRA rates. I tell them to do nothing until they start their new job. If an emergency comes up from last date of employment until they start new job, theyÂ’ll have to pay for the few days & I will reinstate them. Only twice in all the years I have done that has anyone ever needed COBRA in an emergency. Once it was one of the three biggest jerks that have ever worked for the company I do. He was being a jerk about COBRA & insisted that rather than my giving him the letter by hand, I had to send it via certified mail. I didnÂ’t care, I sent it certified mail. He almost made several people quite over the years and tried to divide various departments and was an asshole. It had to have killed him to call me and beg me to put him back under COBRA because his new employerÂ’s insurance had a waiting period and he was @ work when letter was delivered to his home and then he was so sick he couldnÂ’t go pick it up at the post office. My older brother always says that God will get them sooner or later. It was funny that he got it within a week of leaving.

Of course, Obamacare is wrecking these private plans and Medicare marches deeper and deeper into the debt hole while doctors keep getting fees cut so they are dropping out.

196 As a welfare caseworker (my specialty is retroactive Medicaid eligibility to cover hospital expenses in Philadelphia) I've seen the number of applications for ER visits jump through the roof, especially in the past year.  People go to the ER for things most of us would just treat with over-the-counter meds and bed rest if needed.  Individually the bills aren't really that big, but collectively the hospitals are losing a ton of money as most of those bills don't get covered by Medicaid (for one reason or another people don't qualify) and have to eat the bills themselves.

I suspect that a lot of those ER "frequent flyers" are being coached--show up with no ID and claim to be homeless.  From my application caseload alone you would think half of Philadelphia was penniless and sleeping on the streets, and that's just not true.  Another group driving up the costs are illegal aliens who use the ER for every little ache and pain--I swear half of Jakarta is living in South Philly and came here for treatment of chronic medical conditions, and every labor and delivery application I get is for yet another "anchor baby".

It doesn't matter if the specific Medicaid program is state or federally funded, because either way as taxpayers we pick up the tab.  And the cases I reject and the hospital eats?  Well, just look at your itemized bill the next time you go to the hospital--you don't really think rubber gloves and cotton balls cost $10.00 each, do you?  This system is broke and the last thing it needs is Medicare expansion or Obamacare; God help us if those fools in Washington manage to push through "immigration reform"....

We finally started doing EKGs on him in triage and he started using the hospital about a mile away. Lol they called us to tell us they knew what he was doing too.

I could only get a four oÂ’clock appointment with my doctor and it would have been through Boston to the South Shore, which is a nightmare. I called and said I didnÂ’t think I would make it at four. My doctor told me to go to urgent care because she was concerned. I went & they took me five minutes too late. I needed an ultrasound to rule out a blood clot & they shut down the ultrasound machine a few minutes before that. That doctor sent me to the ER and I had to pay them and the $150 ER co-pay. I didnÂ’t have a blood clot and it took weeks to improve.

236 Sven thanks I survive every day I'm. Stage 4 now been that way 18 months have an oncologists that's working miracles for me and others. Sometimes the backwaters end up with the best docs due to the slower pace. We are certainly blessed. I would give the cancer back just to be able to go back to what I love and would gladly pay for the FSA as well. Perspective often changes when the big C is on the books. Thanks again. I don't want to nag him into misery, but I want him around a long long time. Getting older is hard. Men and women seem to react so differently to aging. Even after ~30 yrs of marriage, I find myself unsure of how best to handle some things.

349 Anybody in my area knows that most doctors avoid taking on medicaid patients, so they go to the emergency. Also, some of them are too ignorant to realize that before 10 o'clock at night it would be simpler for them to go to an urgent care facility. If you already have a family doctor and you end up on medicaid because of some change in your circumstances, your doctor will keep you, but they won't take new medicaid patients. This is the problem with gubmint healthcare, doctors don't like being dictated to by bureaucrats with high school diplomas, or even GEDs. One actually told me this. My suspicion is that with gubmint getting more involved in medicine, the best and the brightest will go into other fields and we will be stuck with the kind of people who used to not be able to get into medical schools. Ya know, third stringers.

368 Off and on I do translating at nearby hospitals, mostly in the ER. To get the (govt) job I had to sign an interesting non-disclosure agreement. I can yap all I want to about the most intimate details of a patient's life and condition, I guess, since they don't say anything about that. But I cannot say anything about immigration status or about how the patient "pays." I think it's strange that the main concern for "privacy" here is not to protect the dignity of the patient, but to keep the tax-payer in the dark. Oh....who am I kidding? Not strange at all. Standard procedure for our ruling class.

371 One of the reasons I'm not an ER Doc anymore is because we were getting saturated more and more with medicaid patients that literally cursed and spit at you if you didn't give them narcotics or whatever they wanted and I literally told Admin that when OCare went into effect our patient census was going to double or triple with people waiving figurative OCare cards screaming, "You have to see me I have ObamaCare!". Did admin care? No. They cut funding for the ER. Another reason is I'm goddamn sick to death of assholes telling me how some dumbass doctor somewhere fucked them over while they are asking me for a diagnosis or a script. Insult me and then tell me what lying assholes all doctors are is a sure fire way to fire up my falling level of compassion. If you've never worked in an ER you have no idea what people are like. I have also literally been coding someone and had the trauma door open with a strange patient sticking their head in saying they really need to get going are you busy I have a pain in my whatever. Fuckit.

Actually, you don't have to be that ruthless.  My modest proposal is a government health loan program which like student loans and child support cannot be discharged by bankruptcy.  If you need medical treatment that you cannot afford and you have no insurance, you simply sign the form and the government will pay for that treatment. 



From then on, until you have repaid the loan, the government will automatically take 20% of your income, either out of your paycheck or directly from your bank account.  You don't need to do a thing.  It is that convenient.  If you die before the loan is entirely repaid, your children are not stuck with the bill.  They can have whatever is left of your estate after it has been auctioned off and applied to the loan.



Or, you could buy health insurance.

378 I was in a pharmacy last year and a lady perhaps around 30 years old  (you can guess the race) was asking the pharmacist for assistance picking out some medication for her "sore throat".  The pharmacist said it could be strep and she should go have it checked out by a doctor and there was an Urgent clinic right around the corner where she could get checked out.  But what does the lady do? She says she'll call "da ambuhlantz" because it will take her right to the ER since she had government insurance and they pay for all for that.  So she pulled her smart phone out of her Gucci purse and called 911.  A few minutes later, the ambulance and a fire rescue crew showed up, checked her out and took her away.

It took me a while before I could get my jaw off the floor.
