Doctor's Office Spends Two Hours on Hold Seeking Obamacare Authorization for Surgery

The doctor's office manager eventually gave up, and performed the surgery absent an authorization from the insurer.

Paperwork problems almost delayed suburban Chicago resident Sheri ZajcewÂ’s scheduled surgery Thursday, but Dr. John Venetos decided to operate without a routine go-ahead from the insurance company. That was after VenetosÂ’ office manager spent two hours on hold with the insurer Thursday, trying to get an answer about whether the patient needed prior authorization for the surgery. The office manager finally gave up.

“I’m not a happy camper,” said Nate Zajcew, the patient’s husband. The couple signed up for a Blue Cross Blue Shield bronze plan through the federal HealthCare.gov site on Dec. 16. “I understand it’s just a matter of paperwork and yesterday was a holiday. I can be an SOB, too, at times, but since they’re going on with the procedure, it’s OK.”

And it took a UK paper to report this story:

Hospital staff in Northern Virginia are turning away sick people on a frigid Thursday morning because they can't determine whether their Obamacare insurance plans are in effect.

Patients in a close-in DC suburb who think they've signed up for new insurance plans are struggling to show their December enrollments are in force, and health care administrators aren't taking their word for it.

In place of quick service and painless billing, these Virginians are now facing the threat of sticker-shock that comes with bills they can't afford.

'They had no idea if my insurance was active or not!' a coughing Maria Galvez told MailOnline outside the Inova Healthplex facility in the town of Springfield.
She was leaving the building without getting a needed chest x-ray.

'The people in there told me that since I didn't have an insurance card, I would be billed for the whole cost of the x-ray,' Galvez said, her young daughter in tow. 'It's not fair – you know, I signed up last week like I was supposed to.'

16 Of course, with the huge deductible on the ObamaCare plan she may or may not have purchased, Galvez would have paid the full amount of the x-ray anyway.  The only real question is whether it would have counted against the deductible.  It's $500 out of her pocket in any case.

19 Okay... on the first one: that sucks, but it *does* happen. Even with private insurance, and especially in the first few days of the year. I've worked those phone lines, so I know of what I speak. On the second- that needs more air-time, but it needs to be made clear that the culprit is ObamaCare, not the Hospital. The Hospitals aren't "turning patients away," they're informing them that they'll be on the hook for the whole cost if they're not really insured- and then letting people decide for themselves if they want to be admitted or not.

If they're In-Network with the insurance, they can only charge the Allowable Amount (for an X-Ray, probably $250 or so).

At the very least there should have been a waiting period anyway.   All of my employers had a waiting period before insurance kicked in for new hires.  This prevented people attempting to get hired for the insurance,  only to get major surgery the next week.  Also it allows the carriers time to implement the policy so these types of issues do not come up.  

 

If you were uninsured  all  the previous years of your life ,  a short waiting period should not be a big deal  and should not have been  a big deal for those wanting to implement Obamacare. 

 

Also this would  reduce the loophole created by requiring carriers to insure preexisting conditions.    The loophole makes it attractive to just pay the fine until such time as you might need insurance.

What I find curious is the fact that this lady could not afford to pay $500 for an x-ray she needed, yet she is going to have no problem paying $450 a month in Obamacare premiums, plus the $5,500 yearly deductible. My guess is she was planning on "paying" for her Obamacare policy with a big fat subsidy check from the U.S. Treasury.

Yep. Same for my husband. Who had taken it every day for the past 12 years or so. Never had an issue with "losing" his meds, but they won't trust him to fill it a day in advance. Which makes pharmacies open on Sunday's an absolute must. It's nuts. For the two or three bad apples, make those in pain suffer even more.

One of the things they are not editorially intimidated regarding is the identification of the race of the perpetrators of various criminal outrages. It's strange but true that I often have to go to their UK site to confirm suspicions about the ethnicities of criminals in ugly cases -- information which their US press counterparts have, but refuse to make public.

Also, if these people were so all-fired sick, why were they a) driving themselves to the ER, b) talking to reporters, and c) driving themselves away from the ER? Was he there already when hospice assigned?  My understanding, which you guys are telling is incorrect,  is that hospice is the care provided and is assigned to where you are when the  requirements for hospice arise.  I have not ever experienced someone being transferred  from their home to a facility to be cared by hospice.

My dad was ill for a long time, in and out of the hospital, and when it was determined that there was nothing left to be done he was transferred from the hospital to the hospice. We, my family, made the decision to go to hospice after many, many conversations with the doctors and because of his condition it was determined that the facility was a better place for the kind of care he needed. We were there with him the entire time and the nurses and aides who ran the place were incredibly kind and thoughtful to my dad and to all of my family.

192 My mom was in a hospice home for her last few weeks. It was a good place, and they generally took good care of her, EXCEPT that it was hard to get enough painkillers. That is a big exception. I blame the war on drugs more than the hospice workers. The rules are very strict.

At one point, I asked if she wanted to dip into her secret stash, and she said YES to that. We had some Dilaudid set aside, thank goodness.

The place was $300 a day.

I suspect that she really had no symptoms. When I went in with my heart attack after I finally got through all the illegals blocking the admittance desk I was back with doctors and nurses immediately.

However, under their glossy coating, and despite Sailer's cynicism, what the apps really are doing is making capitalism work. It's just better ways to distribute resources. I had an idea for a business to resell baby clothing. Sure, babies do gross things to their clothes, but they don't usually wear them out. So wash them up and pass them on. That was until I found out baby clothes are hard to source, because people already save and share them with friends and family. Most of these apps are the same thing, but with a wider pool of users.

My understanding of the deductibles is that only the approved charge (say $160 for an Xray) of an in network hospital (was this one even in this person's network?) of the 80% that the insurer would pay of this particular plan for this service of which the patient would be responsible for the other 20% upfront in cold hard cash on the spot, plus a co-pay of some determined amount of say $60 per service, would apply to this plans $5500 deductible. So $160 minus 80% is $124 minus $60 co-pay is $64 towards the $5500 annual deductible. So if they can prove they have insurance, AND paid the monthly premium, and are an in plan provider, they would have to shell out $160 for the XRay. This is just one part of the ER bill which would have physician services, medication, etc. The providers have no intention of getting stuck with un-paid deadbeat bills. And getting mad and storming out is not going to get people care but it may make them stop paying the premiums.

What happens is this (in private insurance, where this stuff all works): when the office flags you as having a deductible instead of a copay, they call on the day of service to "Verify Benefits." Among other things, they get your total Deductible Amount and Deductible met. They also know (if they're in-network) what their allowed amount for a given service will be. So when you get to the window, they look at the most recent numbers (as of when they made that phone call that morning), and charge you based on that. Which, of course, for the ObamaCare plans means "they won't."

239 Sorry you had a bad experience with hospice. At separate times, both of my parents went to a hospice facility for their last few days. The caregivers were tender, compassionate, and respectful. Even though my parents were unconscious, the caregiver addressed them by name, explained what they were doing, and why. I watched one nurse give my Dad some medicine one drop at a time on his tongue. She had to be uncomfortable in that contorted position, but, it was all about him and making him comfortable.

243 The website really has nothing to do with the biggest problems with this.  For example, unless the surgery is extremely expensive, with the bronze plan (and huge deductible) the insurance company isn't going to be paying anyway.  The only question will be whether the $$ the patient pays goes against their deductible or not.  Go will the surgery goes well, with regards to Obamacare, the Zajcew's are not going to be happy, at all.

I remember having the conversation with the Hospice nurses as well as with those at the nursing home where she resided and it was clear that they would not try extreme measures to save her life at that point but that issues such as infections or a choking event would be promptly tended to. It seems you had a nightmare experience. I am, again, so sorry you and your mother had to endure such a place and such people.

Another nail in my large anecdotal experience.   If I would have thought about all my cases , I would have realized that in every case, a relative was available to be at the home also  so a transfer to a facility was not necessary.

It flickers when on a dimmer. Badly. Yay.

Home Depot where I live has been restocking as they are being wiped out of most common bulbs.

I have been also stocking up on vanity and other bulbs as people don't realize these are also part of the ban.

Also those funky vintage edison bulbs are part of the ban so if you like those types of bulbs snag as many as you can because that look cannot be faked by CFL or lead.

Modern dimmers vary wave duty cycle (with a gizmo called a triac) rather than voltage like old school rheostat based dimmers did.

With a filament bulb, the shorter on times are smoothed over by the filament staying hot'ish.  The instant on nature of LED's would naturally make the on/off cycles more noticeable.

436 eCarjack I watched an episode of one of those car theft reality shows. Apparently it's very common for YOUEE to steal a car just to get around. They had a bunch of kids in a standoff of sorts where the kids knew the cops were there, waiting for the kids to get back into the stolen car. So they had no way of getting back home. The little thieves were just milling around, with no idea how to get out of there.

Drivers have to register to get paid, electronically, so they have no great criminal advantage. The riders might be sketchy, but if they look or sound wanky, the driver can just not pick them up.

Reading a couple of quick articles describing the EOs, the House probably would've played ball as they're fairly minor changes.

One of them is a bit confusing to me- he's expanding the definition of ineligible purchasers to include those involuntarily committed to an outpatient mental health facility.  How does that work, exactly?  Someone is involuntarily committed, but isn't held there?

Obamacare premiums, with subsidies, are dandy for the FSA.   Below a certain income level (2x poverty level or something like that) they get reduced deductibles and copays in addition to heavily subsidized premiums.

It's the working middle class, that won't qualify for subsidies or lower deductibles, that will be totally screwed.
