— Ace The doctor's office manager eventually gave up, and performed the surgery absent an authorization from the insurer.
Paperwork problems almost delayed suburban Chicago resident Sheri Zajcew’s scheduled surgery Thursday, but Dr. John Venetos decided to operate without a routine go-ahead from the insurance company. That was after Venetos’ office manager spent two hours on hold with the insurer Thursday, trying to get an answer about whether the patient needed prior authorization for the surgery. The office manager finally gave up.“I’m not a happy camper,” said Nate Zajcew, the patient’s husband. The couple signed up for a Blue Cross Blue Shield bronze plan through the federal HealthCare.gov site on Dec. 16. “I understand it’s just a matter of paperwork and yesterday was a holiday. I can be an SOB, too, at times, but since they’re going on with the procedure, it’s OK.”
And it took a UK paper to report this story:
Hospital staff in Northern Virginia are turning away sick people on a frigid Thursday morning because they can't determine whether their Obamacare insurance plans are in effect.Patients in a close-in DC suburb who think they've signed up for new insurance plans are struggling to show their December enrollments are in force, and health care administrators aren't taking their word for it.
In place of quick service and painless billing, these Virginians are now facing the threat of sticker-shock that comes with bills they can't afford.
'They had no idea if my insurance was active or not!' a coughing Maria Galvez told MailOnline outside the Inova Healthplex facility in the town of Springfield.
She was leaving the building without getting a needed chest x-ray.'The people in there told me that since I didn't have an insurance card, I would be billed for the whole cost of the x-ray,' Galvez said, her young daughter in tow. 'It's not fair – you know, I signed up last week like I was supposed to.'

At the very least there should have been a waiting period anyway. All of my employers had a waiting period before insurance kicked in for new hires. This prevented people attempting to get hired for the insurance, only to get major surgery the next week. Also it allows the carriers time to implement the policy so these types of issues do not come up.
If you were uninsured all the previous years of your life , a short waiting period should not be a big deal and should not have been a big deal for those wanting to implement Obamacare.
Also this would reduce the loophole created by requiring carriers to insure preexisting conditions. The loophole makes it attractive to just pay the fine until such time as you might need insurance.

The Daily Wail is a weird thing.
They are basically a flashy tabloid, absolutely stuffed with cheesy celebrity gossip about shallow people in whom I have not the slightest interest.
But, they're fearless about reporting certain types of stories, and unusually for a UK tabloid, they've realized that in the age of the Internet, they can draw a profitable US audience by covering stories from this side of the pond.
One of the things they are not editorially intimidated regarding is the identification of the race of the perpetrators of various criminal outrages. It's strange but true that I often have to go to their UK site to confirm suspicions about the ethnicities of criminals in ugly cases -- information which their US press counterparts have, but refuse to make public.

When 404Care.gov crashed and burned, you could find a lot of lefties who were critical of Obamacare. But now, they are all on board with how wonderful it is! 2 million people enrolled! Most of them Medicaid!
They insist that people are seeing savings already.. despite the news stories to the contrary and despite all of the anecdotal stories of 200-300% premium increases. If you post a link, you must be posting something made up by the Koch brothers! Obama wouldn't lie!

My understanding, which you guys are telling is incorrect, is that hospice is the care provided and is assigned to where you are when the requirements for hospice arise. I have not ever experienced someone being transferred from their home to a facility to be cared by hospice.

It's not about the (very serious, pathetic, inexcusable, and in a sane political world catastrophic) website problems, or enrollment glitches, or these sorts of stories in the first week of the New Order.
Now that we're clear on that.
It's about an electorate so dumbed down, so lazy, or so shamelessly short-sighted and selfish that the best medical care system in the world, with ridiculous and easily solved self-imposed financial and economic distortions, could be assaulted with 10X the level of the same state-centered pathologies (federal and state). It's about the degradation of the insurance industry into an Argentine-style rent-seeking collaborating cartel. It's about the complete absence of a political opposition that can be taken seriously.
It's about a shortage - a desperate, probably fatal shortage - of people in America who are, in any historically substantively meaningful sense, Americans.

My understanding of the deductibles is that only the approved charge (say $160 for an Xray) of an in network hospital (was this one even in this person's network?) of the 80% that the insurer would pay of this particular plan for this service of which the patient would be responsible for the other 20% upfront in cold hard cash on the spot, plus a co-pay of some determined amount of say $60 per service, would apply to this plans $5500 deductible. So $160 minus 80% is $124 minus $60 co-pay is $64 towards the $5500 annual deductible. So if they can prove they have insurance, AND paid the monthly premium, and are an in plan provider, they would have to shell out $160 for the XRay. This is just one part of the ER bill which would have physician services, medication, etc. The providers have no intention of getting stuck with un-paid deadbeat bills. And getting mad and storming out is not going to get people care but it may make them stop paying the premiums.

And a great big thanks to Paul Ryan for that.
The Beltway GOP completely panicked over the last debt fight -- after a bunch of wildly skewed polls showed they were being blamed for it -- and they folded like cheap card tables.
Their current excuse is, "Well, if we have another debt fight, it'll take Obamacare off the front pages. It would do Obama a favor."
Bullshit. The media aren't putting Obamacare on the front pages anyway. Not with genuinely critical coverage. What coverage there is of Obamacare is reliably of the "ironing out a few glitches" type.
There is a general rule of life that the more and different problems you can pile up on your adversary's plate at any one time, the better. It impedes their ability to react. Remember the old toast? "Confusion to our enemies!"
The Beltway GOP's new rule is "Clarity and simplicity to our enemies!"

The numbers are on age breakdowns and are bad bad bad; he had to work to extract the age data from their raw data so give him credit. One quick number -- for ages 18-35 the gubmint needs about 40% of total signups; it was around 20%. Ruh roh.

The American press will never gain its credibility back. Ever.
And that's the big story of the last several years, the biggest story: the complete subversion and perversion of the once great and free American press to the service of the State. The reason most people have missed this biggest of stories is fairly self-evident. We hear about it second-hand, from Fleet Street in the UK.
Personally, I get the sense we are in the penultimate act of a great national tragedy. What happens in the next act, the last act, is anyone's guess, but in Shakespearean tragedies, just about everyone dies in the last act except for that one guy left standing in the middle of the carnage whose job it is to explain what it all means. If that one guy still standing is an American reporter, everything that comes out of his mouth is a self-serving lie.

Small print leads to big trouble.
On the LED bulb package, in big splashy print: LASTS THIRTY YEARS!
On the back, in tiny whispering type: one year warranty.
My general experience with consumer products is that it's fairly rare on average for them to live longer than 1.5x warranty life.
Also: I bought an LED bulb 60w equivalent for the low, low bargain price of $11.
It flickers when on a dimmer. Badly. Yay.
Thanks again, Republican Party and George W. Bush! Great fucking job!

Manufacture and import are now prohibited.
Stocks of bulbs can be sold until there are no more stocks.
Home Depot where I live has been restocking as they are being wiped out of most common bulbs.
I have been also stocking up on vanity and other bulbs as people don't realize these are also part of the ban.
Also those funky vintage edison bulbs are part of the ban so if you like those types of bulbs snag as many as you can because that look cannot be faked by CFL or lead.

The disrupted clusterfucked price structure was created by:
1) govt interference in the form of medicare and medicaid
2) wage freezes leading to health insurance being a perk rather than insurance
3) the rise of HMOs that refused to pay full price
every time they try to grab the clean piece of shit they find another dirty spot

According to the sales person at Lowes I spoke with, what's on the shelves now will be all there is. They aren't/can't get more. They will continue to sell until their stock is gone. Once their stock is gone, it's gone.

Modern dimmers vary wave duty cycle (with a gizmo called a triac) rather than voltage like old school rheostat based dimmers did.
With a filament bulb, the shorter on times are smoothed over by the filament staying hot'ish. The instant on nature of LED's would naturally make the on/off cycles more noticeable.

Cree has been around since '87 is a big player in LED and I think they will be here to stay.
I have enough incands to last for about 10 years.
I will start buying CFL's or LEDS only when I have to.
CFL's have reached their floor in terms of price LEDS will reach their floor in a few years around $7.00-$8.00 dollars per 60watt equivelant or around 770% more expensive than incandecents.

Heaven forbid our Godking bother with the legislative process as called for in the Constitution.
Reading a couple of quick articles describing the EOs, the House probably would've played ball as they're fairly minor changes.
One of them is a bit confusing to me- he's expanding the definition of ineligible purchasers to include those involuntarily committed to an outpatient mental health facility. How does that work, exactly? Someone is involuntarily committed, but isn't held there?

Obamacare premiums, with subsidies, are dandy for the FSA. Below a certain income level (2x poverty level or something like that) they get reduced deductibles and copays in addition to heavily subsidized premiums.
It's the working middle class, that won't qualify for subsidies or lower deductibles, that will be totally screwed.
