Sean Trende: Administration's Claim that Four Million Have Been Enrolled in Medicaid Under Obamacare is Slightly Inflated (By a Factor of Twenty)

[I] havenÂ’t really seen much discussion about the Medicaid figures. The 4 million new beneficiaries seems to be taking on near-canonical status, even being used by the fact checkers at the Washington Post for evaluating GOP claims.

Shocker on that.

This is odd, because after looking carefully at the numbers cited, the Medicaid figures are the weakest of the bunch. ItÂ’s a virtual certainty that the number of enrollments attributable to Obamacare is an order of magnitude less than the 4 million sign-ups implied, and the number of people [on Medicaid] who would actually lose their insurance if Obamacare were repealed is probably around 200,000 to 300,000.

The problem is identified in this Ezra Klein column (emphasis mine):

“Meanwhile, in October and November alone, more than 4 million people signed up for Medicaid coverage. This number will be much higher when December’s totals are released. It’s hard to say exactly how many of those Medicaid enrollments Obamacare is responsible for -- the government’s numbers don’t distinguish between people who signed up through Obamacare’s Medicaid expansion and those who entered the program through pre-existing channels. But the fact remains that Medicaid enrolled well over twice as many people as signed up for private insurance through the exchanges."

(Note: Klein has just published more to this effect.)

This is really important stuff. The statistics tell us how many people signed up for Medicaid, period, in October and November. The problem is that people are always signing up for Medicaid. Even without the ACA, we would have had people signing up in October and November. Lots of them, in fact: Medicaid is a program that services 60 million citizens, so the number of monthly enrollments that keep a relatively stable population is pretty substantial.

So, how many of these 4 million sign-ups are attributable to Obamacare, and how many of them are just people who entered the system under the earlier rules?

Which brings us back to our number above: 1.9 million total enrollees approved in October or November in states that actually expanded Medicaid. If we are correct in our assumptions above—that is, if 10 percent of these enrollees are due to the Obamacare expansion—then we have an actual estimate for Medicaid enrollment due directly to the ACA’s expansion of Medicaid: 190,000.

That's an analysis of the 1.9 million claimed "Obamacare Medicaid enrollees" in the figures he has; if you double that number (taking account of enrollees through the exchanges) you still only get about 380,000.

That's generous in the Administration's favor. Even with such generous assumptions, the real number is less than ten percent of the claimed number.

And yet the Washington Post uses this figure to "fact check" Republican claims.

Corrected: I got confused about the "four million" enrollees claimed and the 1.9 million Trende has information for and thus could analyze. This resulted in my mistating how off these figures were (by a favor of ten, as opposed to twenty) and general numerical confusion.

I agree to an extent, but we also have to keep pointing out how the gubmint is cooking the books to make it seem like Obammycare is helping people   get coverage who haven't had it before.  It's not.  

 

You have to come at the attack from all sides:   not only is Obammycare taking coverage away from people who already had it, it's NOT getting coverage to people who allegedly "need" it.      Not only is    Obammycare denying treatment to people,   it's giving   coverage to people for things they'll never need.    Not only are   the premiums stupidly high, but the deductibles are so enormous that you're essentially paying to have insurance you'll never use.

 

It's destroying the health insurance and health care industries in this country with absolutely zero    benefits or improvements to offset the disaster.

116 I commented about this sometime back. When Oregon expanded Medicaid years back they had tons of people who they never expected come forward to apply. It was termed "woodworking" (as in "came out of the woodwork.") Many of them were eligible under the old system, and yet for whatever reason never signed up. Maybe they didn't know, maybe they were forgetful, who knows. That failure to account was part of what crushed the system (all the expansions were rolled back.) You could have guessed a similar thing would have happened here. Set aside the expansion for a second, just the constant babbling (and the ability of people to sign up in hospital ERs) will have accounted for a large chunk of Medicaid's "growth." People were eligible before, but never enrolled. All Obamacare has done for them is: a) constantly talk up enrollment (something that could have been done for little to no cost.) b) Make Medicaid even more unstable. So...not much.

There is a limit to the numbers of patients any physician can see in a week.   By reducing complexity of individual encounters, "salami slicing" what should be one patient visit into multiple visits just lets them squeeze a few more pts into each work day.

The result is slightly higher reimbursement for the physicians, without significantly more workload.   Its more of a negative for patients than primary care physicians (who are already largely overworked and underpaid).

The long term repercussion of this sort of thing are numerous, but to see the net effect, just look north to Canada (or anywhere else where physician reimbursement is controlled by the gov't instead of the free market).   The Canadian gov't has a rule that if you're a physician and take one cent in reimbursement from outside the Canadian gov't healthcare system, you're barred (I think permanently) from participation in it again.   So that's the extreme version of the measure "proposed" above where physicians are "required" to take some percentage of medicaid pts. . .in Canada the required percentage is 100%!

Ultimately, you just get longer wait times, and reduced quality of care.  This sort of system also discourages the "best and brightest" from going into medicine to begin with, further degrading the medical system in ways that aren't easy to measure.

140 Sister in-law update for the morning horde: The mass in her heart was larger than anticipated, but safely removed. It had done a fair bit of damage to one of the valves and that took them a couple extra hours to repair. She's in recovery and will be on a ventilator for a couple days, plus they still have to do a biopsy on the mass, so she's not out of the woods yet. The doctors felt that it was rare enough that it justified videotaping the procedure to use as a teaching tool, so hopefully other lives can be saved as well. Thanks again for all of the prayers, it means a lot to me.

This is absolutely correct, and there is no "may not" about it.

If you go in for a "preventative" or "routine" medical exam, that's all the doctor is going to do for you. 

If you then ask about your gout/backache/whatever, they'll say "we can discuss your other problem, but then I have to bill this is as "sick visit" and your insurance may not [ie almost certainly WILL not] cover it.  Do you want to do this?"

And don't blame the doctor here.  Reimbursement for routine checkups barely covers their expenses.  If they bill "sick" visits as routine ones, not only is that medical fraud (underbilling is fraud just as overbilling is), but they'll go broke.

I had a nice young lady worked in my shop up in Indiana doing interior work at the yard.  She looked a lot like Lena Dunham and weighed maybe 135 pounds. She lost about 145 pounds between the age of 19 and 23.

I guess saying she looks like her is a little misleading she had dark brown hair worn in a traditional conservative Indiana hair style.

Thing is she was not under the impression she was a sex symbol.

Which is the right judgement call, Lena honey work on the writing God you mom and dad and your environment just said "no."

Like I said in another post, Dunham's stills look a lot better than video of her does.  I was actually kind of surprised when I finally saw a video of her at how bad I thought it made her look in comparison to her stills.  I suspect that there's some very careful positioning in relation to the camera going on, in addition to the usual photoshop fun.

There is a difference between Medicaid and Medicare. I have pointed this out several times. In Medicaid the state can seize all of your assets to pay for your medical treatment. Things like your house, car, cash, stocks and bonds 401Ks - basically anything you own beyond the clothes on your back. Medicaid is not a free meal - it is intended only for people who are so desperately poor as to have no assets to seize. Anyone who believes that it will do what you say is in for a rude awakening.
